Name change for PR?

Hi!

i moved to canada almost three years back. after i got married i changed my last name. i now want to add my maiden name as a middle name and i have no clue what to do? I would not have enough time to get that changed in India (where i come from). I would appreciate if someone could tell me what i would need to do to get the name changed on my PR card and my indian passport. I am a PR and i intended to stay a PR.

You apply for a new PR card, follow the instructions here: http://www.cic.gc.ca/english/pdf/kits/guides/5445E.PDF  As for your Indian passport contact the closest Indian consulate and follow these instructions
