My Surname in the Medical Examination Report is incorrect

Hi,
I recieved medical examination form but my surname was incorrect, should i send it back to immigration office to correct it or do the medical examination then send the report to be corrected.

Is it not clearly mentioned in the letter that if any of the information in the medical form in incorrect, contact CIC immediately?
You need to contact CIC first. Once you get the corrected forms, you have to do medicals

Thanks, How can I correct my name in the medical request? Should I send  the copy of the passport to them or contact them by E-Mail ?

I guess they have your correct name on file and in all other correspondence. If that is the case, an email should be enough.
