I am getting mixed signals from the site and the checklist and when I called CIC. When I phoned, I was told we didn't need to attach the "Statutory Declaration of Common Law" since it didn't apply to us. I guess my question is the same for the "Use of a Representative." We aren't using one, mainly because they cost an arm in a leg. Do we NEED to send that form still and just leave it blank? Or is it better to leave it out fully?

Sorry, mailing this tomorrow so it's urgent.

I included them and wrote N/A across the top and marked N/A on the checklist beside those items. Not sure if it is necessary, but I figured it would at least be clear.

We excluded them and wrote N/A on the checklist which we included as a cover page. However I've seen others on this board include them and simply write N/A. So either option seems to work.

Thank you for responding. Last question as I don't want to start a whole new thread. I just paid my fees online and they ask for a client ID. I have no idea what that is. Looking through the sites now for info, but help would be appreciated. Or am I not supposed to write on the receipt, and leave it to them to fill out? I am confused.

If you haven't had a previous work permit or visitor record, you won't yet have a client ID. So I guess you would just leave it blank if you don't have one.

Good luck on mailing it off today!

Thank you for that. I ended up finding the answer late last night. I will probably start a thread once I know the paperwork has been received. It's an absolutely nerve wracking feeling.
