Hi! Thanks for this wonderful site.

I received an LC2 Application for Permanent Residence in Canada.  I can't find the Checklist, though.  For those who received the same form, did you receive the checklist?

Another question, where can I download the LC2 form?  I tried searching for it on the Net but to no avail.  Can't find the form bearing the LC2 code.

Lastly, shall I include the negative of the photo required?

I know there are kind souls out there ready to help. Thanks and God bless.

The checklist comes from the Region Specific form.  Where are you immigrating from?  What program?  Spousal?

I believe LC2 applies to live in caregivers.

I don't really know anything about that. Is what you need here?

thanks mitamata... you're right... LC2 applies for caregivers... was not able to find the form bearing LC2 in the address you provided... is there a need to send the negative of the visa photo with the application form? I'm wondering why my application kit didn't come with a checklist.
