oh my. so we already have all forms printed out for my hubby's pr app under spouse sponsorship. and all forms have his name written down as:

LastName, FirstName MiddleName

the middle name is his mom's maiden surname. we have included this as part of the given name because  there are no spaces labeled for middle name and the forms say put down name exactly as it is on passport (which includes middle name)

then we just got back his medical report imm 1017 and his given name did not include his middle name.

will this be a problem? thoughts please? thanks

My experience is exactly parallel to yours.

The medical examiner did not include a middle name.

No issue.

Yes, there is no issue about this. If I were you, I will not include the middle name as one of the given names. I noticed this on my canadian born common law partner that they initialized other given name when they write the whole name. Which to other nationality, it means that it is the middle name. So I figure that when they write canadian names, they don't really include the middle name (maiden name of mother). One good example is the ecas update (application status online), they did only put my first name and my last name (surname). I believe they have enough when you put your mother's maiden surname on one of the forms.... Hope am clear....
