Ahh Urgent form filling advice................PMM et al

Guys,

Just had confirmation from CHC that we need to get our medicals done (Woohoo - anyone know how much they cost in the UK?), and also to complete a supplimentary form on my common law partner for PR.

Now the form is the IMM 5406 F and the confussion lies with the wording for section 1!! Does the form need to be filled out by her - as the applicant with me as the common law partner and then details of her parents below OR do she fill in the form with me as the applicant and her as the common law followed by details of her parents below??

Sounds like it should be straight forward as I have always been the main applicant throughout the process, but the wording on this form is clumbsy, and its not obvious as to how it should be completed.

Help please.
