Are you in canada? If so, they advise you to send it to Fort Erie.

Hi Tithi99,

Yes I have requested a receipt from my doctor's office. It acknowledges that I have passed my medical examination, also provides the contact details for the doctor.

Cheers !

You seem did not understand my last replies or didn't see it. PRTD cannot be issued from within Canada, it is equivalent to a visa and must be issued by a visa office outside Canada. You should contact your nearest visa office back home.

You can confirm this by calling CIC Call Center 18882422100.
