Is the Medical report handed over to the applicant/sponsor or it is sent to the Consulate/Embassy in question?

Mailed to the Medical office responsible for the processing office.

The results are entered into an online system so that the office processing your application can view them and make a determination.

But do I get a copy of the Report or just need the payment receipt for my documents?

You won't get a copy of the report.  You send a copy of the receipt to CIC to show them that you had the medicals done.
