Don't you have the copy of the medical form for the second medical?  Call the visa office, explain you did the medical before you received the form, and ask if you can attach the copy from the second medical to the form they sent, as proof you have re-done the medical.  If they find it acceptable, send them the forms together.  Ensure you complete or sign as required on the form they sent you.

If it is not acceptable...well...you just have to re-do using the form they sent you.

I mailed the second medical form to High commission which was given to me from DMP.
so the question is why are they asking for it if they have it?

Did you send it by a trackable method?  If so, advise them when it was received by their office.
