Hey guys. A quick question: I received my request for my medicals. However,  found some mistakes on them regarding my name, my son's and my wife's. They might not be such a big issue, but I just want to make sure

In my application I put does the following: The names bellow as just used as sample and they are not my true name.

My Name: Marcos Oliveira     Last name: Da Costa
Son's Name: Kyle Oliveira   last name: Da Costa
Wife's name: Marissa Oliveira Cabral   Last name: Costa  (She does not use "Da")

In the medicals I received it says:

My name: Marcos Oliveira da   Last name: Costa   (They removed "Da" from last name and out in the middle)
Son's Name: Kyle Oliveira da   last name: Costa  (The same here)
Wife's name: Marissa Oliveira Cabral Cabral last name: Costa (They repeated here middle name "Cabral" Twice).

Are the above an issue? How do I fix them?

I had kind of same problem where my surname and given name were wrong, so Doctor corrected the mistake on my medical form (checking my passport) and asked me to send photo copy of passport with Residence Fee so CHC can correct it. its also mentioned on the medical form (information side) to contact them if mistakes found., hope that will help.

Canadian PR use to take 5/6 yrs but we were happy. Now it takes max 1 year but we can't wait and before filling application start asking, when will i get med/Passport request? Do your home work, check forum previous pages read CIC site and respect seniors

Not a problem.
Just ask your DMP to amend them putting his signature. He'd check your 'original' Pps & do the needful. Also, concurrently send an email to CHC about the same & that the DMP is taking action to correct it.

The only problem is that I used to check my application status on line on e-cas using my last name as: Da Costa as it should be. Now, I can't. I can only check using: Costa.

What is going on?

I think I would leave it like that. It might delay the whole process if you ask them to change. Again, try to have your DMP correct it.

But that time i went ahead for giving the medicals of my 6 months old boy. i have mentioned the same in the covering letter while sending the docs to ND with RPRF and original PPT.
is this that is affecting the case delay...

What do you mean it caused the delay of your application process? Are you saying that you went to do the medicals and after doing it, you sent a cover letter explaining it along with the medicals and that is what is delaying the process?

Shouldn't be a prob. However, u should have asked your DMP to set that right on the forms itself. Send a repeat mail to CHC informing about the DOB correction.
