I'm sure there's a simple answer to this....

So we do the medical before we apply. The DMP will give us the original Copy 2 of the medical report, Copy 1 will be sent by the DMP to the appropriate medical office for processing.

I just don't understand how the Canadian Embassy will even know who my partner is if we haven't applied yet.

Maybe it's a silly question....

Well you send copy 2 with the application, and it has your name and birth date.  So bingo, they search the IMRC where the medical results were sent by DMP and sure enough, there it is.

its a mater of few seconds for them....they just need to scan the code on the copy 2 given to you by the DMP and the report with the same code will be available immediately....
