MEDICAL FORM - SECTION C , PLEASE HELP

Hi
I am getting my medical done tomorrow. I wanted to know, when i go
do I take -SECTION C medical form.
- my passport
- 4 passport photos. ?
Is there anything else I need to take?
I asked my DMP, he didn't seem that certain.
Please help.

Appendix C
Passport
If you use glasses, take them.
If you are on medications, take a list.
My DMP wanted 5 photos.
In Ottawa the medical, blood work and x-rays were all payable in cash.
