I had mine done in North York as it's convenient to me and they speak my language.

$175 in total, breakdown and the flow is as below:
- $130 paid at doctor office (form & questionnaire, eye & hearing tests, weight & height, blood pressure, simple body checking, blood test and urine test).
- $45 paid for chest X-Ray in a nearby building (3 traffice lights away, the York Radiology Consultants, 103-1100 Sheppard Ave. East, Willowdale, ON M2K 2W1).

Overall it took less than 2 hours if I remember correctly.

If you cannot see him on the list then he is not approved and your wasting your money and time as your results will be rejected duhhhhh!  Stick with the list of approved DMP's only

thanks, i found one at pape and danforth that does it on a walk-in basis and is open saturdays!
