Author Topic: MEDICAL DILEMMA for Leon, Pmm, CPA2CANADA and other Senior members  (Read 580 times)

My Wife and I are doing our medicals tomorrow.
On my wifes Medical Report Form IMM 1017 her forename is written as Sandra.
However on her passport it is written as Sandra Mary.

In the Medical instructions letter it states (QUOTE)" It is imperative that you verify the following information on the Medical Report Forms: SURNAME, FORENAME DATE AND PLACE OF BIRTH, *censored word* and MARITAL STATUS must be EXACTLY as stated on the passport.
You must immediately advise us of any inaccuracies and provide copies of the passport(s).
The smallest discrepancy could create significant delays in the processing of your application".(END QUOTE)

Are we ok to go ahead with the Medical exams tomorrow.and will the name difference need to be rectified.

Gerrybyrne.....had the same case here..in form 1017....marital status was not written.....I proceeded with my medicals and later on I sent a specific inquiry to London Office regarding the ommission of my marital status.....they replied after a week telling me that the forms are still valid.....meaning....we just need to tell them the discrepancies via email and they will correct it accordingly.....but for the meantime I think you can go ahead with the medicals...

CPA, Thanks for the input. I was thinking it would be ok, but its nice to know someone else had a similar situation and got the answer to it. I sent them an email earlier.
