Hello,

My husband will apply as the principal applicant. After the marriage I took my husband's surname.

My question is: does my marriage surname is considered as my "Family name" (as stated at the "Generic form" and "Additional dependant form"), and I should write my birth name under the "Have you ever used any other name?" question, or is it vice-versa?Huh

i am not sure...but i think surname will be ur husband name...nd they have also asked bout ur birth name somewhr else in the form.

Yes, it is your family name (surname) now.  You write your maiden name "used another name"
