MANILA VISA OFFICE REQUIREMENTS

I have a lot of question regarding the above-mentioned subject:

1. Submission of forms: Photocopied of ur initial app to the CIO or you need to sign a new one?
2. Need to pay another processing fee to the VO or not anymore since you already paid 550dollars to the CIO?
3. Photocopy of Birthcert from NSO, ok? or Original?
4. Police renewal needed? When the date is still valid when you submitted your initial app to the CIO... what is their basis to determine if its still valid? Ur date of submission to the CIO or VO?

Please someone help me and share their experience here.. thanks!
