Author Topic: Lost Original Medical Report Section- A  (Read 368 times)

Hi there,

I am planning to submit the spouse sponsorship application by end of this month, all the dox are ready and my wife did her medical two weeks back. Now the problem is that she sent the papers to me through courier and the medical report section A is lost in between. I have photocopy of medical report section A but the original is lost what should i do ?

Will CIC accept the copy of medical confirmation or my wife have to do her medicals again?

Please advice

We never even got the Form A from our doctor! I just sent in a copy of the receipt for the medical and the receipts for the chest X-ray and bloodwork and they accepted all that. Just send the photocopy and any proof you have, it should be okay. They get the results from a centralized database anyway, so the results will still be there, even if you don't have the receipt.

Thats what we did as well,we also put in a print off of the delivery to london of the medicals as proof that they had been delivered there and who had sgned for it. not sure if you get an email from every DMP but we did with the tracking number on, so was able to see when it got there.

If you want to, you might be able to order another copy. I ordered that medical form after the actual appointment because they had only given me a receipt (not the outland form on the checklist). I had to send in 2 or 3 photos and maybe $30? You could always call and ask to see if that is a possibility.
