LMO Process

Hi all,
I am working with a company who are willing to sponsor me and apply for the LMO. Where should I start? Where can I get the forms and correct paperwork fot the application??
Any comments appreciated!!!

At what point of the process do I apply for a visa extension for my common law partner? Is it when I am sending my approved LMO to CIC?
