Hello,

I have 2 daughters & they are 3.10 yrs & 11 months respectively. We are currently in US & planning to go to canada this month end.
I heard about the immunization form but cannot find any link to download this form.

Can someone please share the correct link?

do you have immunization report by your baby pediatrics? keep those with you. it's ok it will accepted. immunization report mostly required when you baby will join canadian school. strickly rules for those whose baby want to join school and get admission in school when the new immigrant will land. so if your baby not going to join school then its not mandatory, you can get it from Canada too.

I'm OMM anyway, no worries you can get it from Canada Public Health Student Immunization of your city where you will be going to land and stay.
you need this report when your child going to join school admission time. if you don't have this record your child school settlement worker will help you.

just keep all USA immunization report upto date and keep all with you.
