latest forms for spousal

does anyone here fill up the application form already and suddenly showed that as of july 18 2011,need to change..do we need to change all the form or just the IMM0008? and i used my husbands last name when i filled up but in my passport still my last name when i was still single..i dont know which one to use for filing?

I had actually sent my husband the old forms before 18 July 2011 but he hasnt submit the application and we only realised it early August of the new forms, so I just redid the IMM0008 and resent him that only, the rest in the immigration doc are old forms unless you have more than 5 dependents, then you use the new additional dependent forms as well. (he has a new form (first one) as well as Sponsor)

About the last name, not sure, but I think from what I have read, you must ensure it matches the passport so either you update the passport or the forms. maybe some of the seniors can you assist you better here.

so what did u used then? coz i used my husband's last name now..i think thats fine

My husband submitted with new forms for IMM0008 since it has the barcode hoping it does expedite the process. As for the name, I don't intend to change my surname to my husband's so no issue for me as the one i put in the application is the same as my passport,
