Because any one who is over 18 and included in application needs his / her separate consent. As your wife is over 18 and included in your application she will need her own IMM5475. In your IMM5475 your wife will sign as your spouse and in her IMM5475 you will sign as her spouse. Make sure relative/friend's details should be same in both forms.

I advised the same in your PM too.

Any children who are under 18 won't need to fill IMM5475.

About 2..............in the bottom of the form, there are two spaces for signature, one for PA and other for Spouse. Why do we need two separate forms and ....?

1. my email address so CAIPS result directly reach to me without any delay or relative email address.
2. two email address are allowed ?
3. will they send Hard copy also or just softcopy ?

Well the IMM5563 form (page 2) says to write details of dependents? Didn't you include children in your FSW application?

As you have already emailed hopefully you will be ok as they are less than 18 years of age and their consent isn't required.

Separate consent form is required from spouse because her information is included in file and if you don't provide her consent form they can stop releasing information relating to her/him. I have seen it carefully the IMM5563 form.

- Who needs to submit the Consent form

A consent form is required of each adult individual (over 18 years of age) whose name is mentioned in the immigration application. For example, as a principal applicant you should enclose a copy from your self and if applicable a copy from your spouse or adult dependants.

If the consent form for all individuals is not provided then CIC will sever information related to the missing consent form.

No dear it should be all dependents either 18 or above 18 if they are included in FSW application. Just in case of above 18 one needs to also tick consent included.

Thats great...You are done. Make 2 copies. Keep one for yourself and fill other and then scan with your application.

Name. Name of payer i-e your Name.
Address. Address of payer.. Your address.
Date of birth . Client date of birth means your date of birth if you ordered GCMS for yourself
Client ID. UCI number. (yours if you ordered GCMS for yourself
Post code. Yours as you paid online
Area code and telephonoe no. Yours as you paid online

i pary it will work but CAIPS /GCMS  issued only for canadian citizens living in canada.

It does work. It worked in my case and I am not living in Canada and also in case of another applicant it worked and he is also not living in Canada. Thanks

He / she can give any email but it is asked in requester's contact information and it is only reqester who is supposed to order so preferably requester's email address. Thanks

He has already filled and sent the application. You are bit late responding here but just for your info in

3. Preferably tick both CAIPS and GCMS and they will send whatever is available CAIPS or GCMS for his file.
