Cocknbull u r great.Thank you very much.Dear plz help me again How can I fill IMM5475 form question number 5) Your designated individual's full name. This is for my relative who is living in Canada or others.And other thing is First form have to send this address

Ottawa,ON,K1A 1L1. Second, IMM5475 form have to send CIC Nova Scotia,Sydney.Because bottom of that form written "If you have already submitted your application:  Send this form to the office where you submitted your original  application"".I got 1st AOR so I have to send two forms two different addresses or same address.please give me solution i'll be grateful to you.my relative will order for GCMS by post.

2) what should i tick - Access to information Act 5$ or Privacy Act

4) Scan both the forms 5563E require uncle sign and 5475E  sign by both me and spouse and Email it to my uncle

2) Tick both access to information act and privacy act.

3) Tick both CAIPS and GCMS

6) Ask your uncle to include copy of his PR card or Canadian passport and copy of his current bill or bank statement.

Hi!

Yes you can if your card works there. After payment take 2 print out of receipts. Keep 1 for yourself and fill other and scan it with your application. Thanks

now question is that - on receipt

 1) Name ( whose name / applicant / relative )
 2) whose address/ DOB/ contact no etc ( applicant / relative )

 i had tried it now but on receipt it mentioned 1-888-242-2100 Hours ( Local time ) : 8:00 - 16:00 ? what does it means // on receipt -payment was accepted date of payment Mar 20,2012 2:XX AM .Approved Thankyou !!

Thats great...You are done. Make 2 copies. Keep one for yourself and fill other and then scan with your application.

Name. Name of payer i-e your Name.
Address. Address of payer.. Your address.
Date of birth . Client date of birth means your date of birth if you ordered GCMS for yourself
Client ID. UCI number. (yours if you ordered GCMS for yourself
Post code. Yours as you paid online
Area code and telephonoe no. Yours as you paid online

I have emailed asking for GCM without mentioning details of my children (all under 12 years) but with consent and details of me and my wife only, is that ok ??
