landing form filling guidance

dear friends
I am asked to fill in b4a form and get it stamped at Toronto airport. Basically the form has columns 'item article', 'description' and 'value for duty'. There is also a check box 'list of goods to follow' which I can enable adn disable.
Questions:
1. We are taking along with us (checked-in 23kgsx2 per person & cabin baggage 23x1 person) essential clothes, few utensils (e.g. plates, spoons, few cutlery itmes, etc), few home furnishings, wearing few jewels (rings, chains), few thousands of C$ - TO ACCOMPANY CATEGORY.
2. We will then bring through Cargo major utensils, clothes, home furnishings, etc. We will bring jewels in person - TO FOLLOW CATEGORY

a) Is b4a is the only form  i have to use for both 'To accompany' and 'To follow' category.
b) One form is not sufficient, can i take copies and use it for subsequent pages.
c) can I starting all itmes in sequence or i should use one-one form for categories i.e. for clothes, vessels, jewels etc.
d) what is the value i have put under 'value for duty' column.
e) If a specimen of filled-form is available that would be great.

Welcome to Canada!!

You don't need to fill up forms for the items you carry with you.
You can verbally answer about the contents to the CBSA while landing, when asked.

Regarding unaccompanied:
1. We are taking along with us (checked-in 23kgsx2 per person & cabin baggage 23x1 person) essential clothes, few utensils (e.g. plates, spoons, few cutlery itmes, etc), few home furnishings, wearing few jewels (rings, chains), few thousands of C$ - TO ACCOMPANY CATEGORY. - You can talk to CBSA about it - you have dress and some personal items for home.
2. We will then bring through Cargo major utensils, clothes, home furnishings, etc. We will bring jewels in person - TO FOLLOW CATEGORY

d) what is the value i have put under 'value for duty' column. : Just mention for each group approximate value like CAD 100 or 150
e) If a specimen of filled-form is available that would be great. No need. Take it easy. Example:

Assorted ladies dress  - 20 pairs:>:  100 CAD
Assoted Mens dress - 10 pairs :  100 CAD
Assorted children dress - 10 pairs: 75 CAD
Assorted shoes/foot ware - 12 pairs 80 CAD
Home furnishing various type - 10 ea : 100 CAD
Assorted cookware - One/two box: 150 CAD
add any thing more....
