baka ung appendix c lang ung sinasabi nilang referral. kasi nung tumawag din ako hinihingan ako tapos sabi ko ung appendix c lang ung pinapadala. indicated naman un dun sa kit diba? tapos nung nag walk in ako ok naman

  @   Jovy    .. really when i called the medical office here in cebu(one of the practicioner). they asked for referral from canadian to know what part to be examined.. really confusing because i had read one of the forum that they just directly go to the examination eventhough they havent passed their application as what u said too...What I'm gonna do??? so confusing.. and By the way what to bring to the doctor aside from your passport only the appendix c??? salamat sa reply

Share nyo naman guidelines/advice for medical. My sis. just informed me that she got my med request 3 days ago.. Sobrang limited lang time ko kasi ala ako sa pinas, di pako nakapag file ng leave and maybe 1 week lng allowed.

Are you applying under Spousal Category?

I was examined in Cebu (Cebu Doctors to be specific). All I did was called them for schedule and then I was told to bring the Appendix C, 5 passport photos, passport or valid id, and I guess that was like 4000 pesos (cannot remember).

First day is laboratory & xray. Second day, physical assessment done by the doctor. And then after that you will be given a COPY 2.

- 2 copies of filled-up Appendix C
- 9 copies of passport photos
- 1 photocopy of passport (biopage); bring original passport for verification
- 1 photocopy of valid id; bring original valid ID for veirification

Things done:
1. Submit filled-up Appendix-C to the information staff
2. Fill-up and answer related forms -- index card, 1-page form
3. Submit the above requirements
4. Pay the required fee
5. Extraction of blood sample
6. Collect urine sample
7. Sign related forms/docs
8. Xray
9. Physical Examination by the DMP -- Q&A about your health history, eye test, height, weight, BP, breast checking, anal check, ear check, tonsil check, thyroid check, etc...
10. Wait for the Copy 2 Medical Report with attached Appendix-C to be given to you

ah okk salamat.. i tried to call them yesterday 5:30pm nobody answer maybe they were closed already i will just tell them about it about ur experiences especially for sthomas were same here in cebu... sabi pa naman huwag kumain matamis at drink.. water lng hehehhe.. kong ma detect nila if my sugar is high di ba ako makapunta sa canada.. hope not. nag heal naman mga sugat ko kaagad. kasi mahilig ako sa matamis... salamat sa instruction gud luck to all
