After the examination
- However the DMP will also provide you with the photocopy of the completed medical examination forms for each applicant. You should submit these to our office, along with a photocopy of the passport which was presented at the time of examination for each applicant.
What form is it referring to ..when it says "completed medical examination forms" and what photocopy of the passport "You should submit these to our office, along with a photocopy of the passport which was presented at the time of examination for each applicant."
For me, DMP didn't do anything on the passport. What photocopy it is refering too? Please let me know

BRo DMP will provide you with the receipt of the payment of the medicals and also the xerox of the first page of your medical application form.
send both and regarding passport copy, I sent complete copy of my passport along with original passport |

Hi, friends
I got e mail on my request for reconsideration of my application from the officer. He stated that the only tool I have is to apply again. The office does not consider any additional docs after decision has been made. Although one guy advised me to do that because his case was opened after request.
So, guys that is my experience. But I am going to apply again. Besides I know my mistakes. And I will be prepared more for that.
I have one question. Do I have to collect all documents (notarized copies of translated diplomas and so on) again for new application? Or filled forms will be enough because they already have the same docs?
