Our time lines are as per Islamabad VO. (27 Month)
23 Month on CIC website is for applications submitted by UK residents and others eligible to apply in London Office. I have also got the same info from my lawyer. (Those who got AOR in Aug have to wait 4~6 months more for meds) and 27~30 months from AOR date  till visa issuance.

Only after full submission of documents to visa office.

yes only medicals and for visa 26 to 30 months OR may be 36 months in some cases.
