r u married? you are applying with your spouse? did you as principal applicant and your spouse filled up the form one copy each both you?

yes I am married... spouse and child details have to be in Generic form 0008.

well i don't know they put the UCI number on my form. may be for reference again. No they did not email me any thing I just recieved the pack ...MAN i cannot take care...some one at CIO has to look at the applications with care...because of their stupid mistake some one can loose a position from the quote...any ways I believe I still have a chance in the quota... so I am sending the application back again I have enclosed the letter as well to let them know that the form was their but u didn't see it...

and I wish best of luck and congrats to all who has received their PER and AOR.

actually I confused regarding that form ( additional family member ), the spouse required to fill up separate form? if she required where she can put her name?

Dear Ali,
Let me share my story but before that let me introduce myself i'm a silent member of this forum and read valuable post from all members especially senior members like shamloo,Djoker, mahali, FSWCIO and offcourse the great one aifarooqi.

I sent my application to CIO in mid 2010 and after 10 months without any acknowledgement of receipt they returned my application and in email they mentioned that processing fee will not be refunded as the application was placed into processing, the reason they given was my total points were not enough to qualify. I was so devastated and angry because they said the points for relative in Canada can be awarded as I didn't provide sufficient evidence. As per my calculations my points were 71 and as per CIO assessment points were 66 so naturally being short of points I failed and was blaming CIO for not checking the application properly I planned to never apply for Canada again and was seeking alternative, after few months later one day I rechecked application and figured out CIO was right I provided some of the evidences as proof of relative in Canada but none of them was having our parents name in there only my brother name was there, although it is required to have such a document which must have atleast 1 common parent name. I provided copy of my brother's Canadian passport, his hydro bill, citizenship card copy so I figured it was lacking from my side. I would suggest everyone to read the instruction guide several times before filling up application and make sure all the required documents are there otherwise you will end up with wasted time and money.

sorry to hear your story. I am not in a rush ...they have returned my application and fees too...so there is not harm in sending the application back again... as I know the form they are requesting is inside the application. Dear Muhammad Ali...sorry to hear about ur application..one suggestion...have u written all the names in ur native language also in additional family document???this is the only reason they can send it back( its my opinion only)...cz i have seen my friend forgetting this on the form...so the form is considered to be incomplete...
