If I may ask are you a female applicant? kaninong authorised doctor/clinic ka nagpunta for the medical.. I noticed karamihan male doctors for manila/makati.. and can you share how long inabot ng medical? anything I need to know? sorry sa questions ha.. super new lang ako dito sa forum. hinanap ko lang agad manila applicants..


p.s. how do you that na naqu-quote un message ng gusto niyo replyan? sorry newbie hindi ako nagbabasa...

hi sis! i went to st. lukes, girl naman yung doctor na nagcheck sakin kahit na lalaki yung nakalagay sa list of accredited. i have the same apprehensions kasi ayoko ng guy doctor. when i went to st. lukes extension, the name pala indicated on list is parang the head doctor. parang sya yung pinakaboss so i dont think he does any physical exam pa doon. yung sa akin babaeng matanda. i went ng thursday afternoon. super bilis kasi ako na yung last.

uyy talaga 20 days after registration? galing ako Manila city hall last Jan 26 and nag-order ako ng certified true copy ng MC namin.. xerox lang siya na may tatak from city hall pero sa bandang baba ng MC, sa number 22. REGISTERED BY THE CIVIL REGISTRAR nakalagay Dec 29 2011... Hmmm.. does that mean nsa registrar na ng Dec 29 and possible na na-transmit na un sa NSO? Ang sabi lang sken sa city hall, kung Dec daw un wedding, dapat nsa NSO na un last January kasi every 2 weeks sila nagfo-forward sa NSO...

btw, anu un SECPA copy? iba pa ba un sa certified true copy? in case kasi susugod na ko sa nso main hehehe

and sis, nagpa-medical ka na din ba? san ka clinic nagpunta? plan ko na din punta soonest!

ay question pa pala, did you use your maiden name sa application? gusto ni hubby maiden name gamitin ko sa lahat kasi kung magpapalit pa daw ako sa married name like sa passport, '10 years' na naman un!





I suggest use your name as it appears on your travel documents.

hi merger, as i can see you know na how to "quote"  Grin  Pag nasa NSO ka try to be upfront when you talk to those  lazy and old fat NSO staff sitting around sa counter, very typical of the filipino government people, remember pag medyo mahina ang approach mo sa kanila they won't mind you.  Mas bibigyan ka pa nga ng direction ng mga security guards standing around the area.... ask them too.  Give them a very precise reason that you need the expedite NSO MC for your application to Canada, that your spouse is sponsoring you, they will ask pa nga stupid question to you if you already have the visa??? how can you have a visa eh you haven't submitted pa nga the application and still completing the requirements why you are there at NSO.

And of course im a Female applicant (Ysabelle nga eh lol) i had my very 1st medical when i first went to Canada to work year 2007 sa Nationwide sa Makati i was attended by a female young doctor, they seems to be more gentle as per experience than the 2nd time i had recently nitong 1st week of Jan. 2012 sa St. Lukes extension in Bocobo Manila,  female old doctor naman very rude nga eh during the physical exam...  but anyways just bring with you P4,200 if you want it in St. Lukes, will only take 2 hours for the entire medical process.  Don't worry puro female doctors naman eh so nothing to fear...

hi sis, i got married DECEMBER 11, 2011 ganyan din ako ka depressed last month, i don't know kung pano at kung saan hihingi ng tulong para mapabilis yan nso marriage cert.. palagi ako pumupunta sa Manila city hall para makiusap na maiendorse ung cert, but they kept on saying 6 months.. i never lose hope, punta pa din ako ng punta dun, then one day nag-ask ako dun sa babae na pinagapplyan namen ng marriage license before, nagask ako sa knya kung na submit na sa NSO and she told me na TRANSMIT na daw Jan3 pa Smiley

My advise is kung sa Manila ka naman ngpakasal mabilis sila magsubmit ng documents, ask mo nalang ung lady kung kelan kau naregister sa local civil registy and kung kelan na transmit sa NSO. WAG KA MAKINIG DUN SA MGA NEGATIVE NA TAOSA CITY HALL KASI MADEDEPRESS KA LANG Wink
after 20 days of registration pwede kana magrequest sa NSO, and don't apply online, GO DIRECTLY SA NSO MAIN EAST AVE BRANCH, and pakita mo ung copy na makukuha sa city hall, kuha ka ng SECPA copy may tatak un sa likod kung kelan na transmit then dalin mo un pag nagpunta ka sa NSO.
i got our marriage cert last Jan 24, pero feeling ko mas makukuha ko pa cia ng maaga kung dumiretso ako agad sa nso, nauna kasi ako kumuha sa outlet ng SM ang tagal tagal naman feb 2 pa release..
in god's grace naman we completed the requirements na, and i already send it to mu hubby, tapos papasa nia na this week sa cic, february batch na kame:)

goodluck sis and i hope makuha mo na yan sa NSO main ha,
it's so depressing ung pagaasikaso ng requirementsa dito sa pinas, ang haba ng waitng period..

I use my maiden sa application forms namin, since my very recent passport is also reflecting my maiden name.  Will eventually change to married name once settled na with hubby in Canada.

I use my maiden sa application forms namin, since my very recent passport is also reflecting my maiden name.  Will eventually change to married name once settled na with hubby in Canada.

I also used maiden name since my passport and PR docs are still under my maiden name and the forms say "exactly as shown on your passport" . =)
but dun sa IMM1344E,  I checked YES under "have you used any other name?" and I put my married name there =)

Hi Ysabelle, yup this is required. I too didn't know about this when we were preparing the docs, then nagbasa basa ako ng MANILA threads dito and I noticed laging namemention yung AOM na yun, so I double checked the Appendix A checklist (Philippines) and ayun nga andun nga siya. I also missed it the first time kasi ang nakita ko lang dun eh "CENOMAR" kaya hindi ko na binasa yung katuloy na words eh nakalagay pala dun "or Advisory on Marriages", hehehe. Kaya ayun nagrequest na ako. But don't worry, if you lack this naman, as long as you gave your NSO MC, they will ask for the AOM na lang kasabay ng PPR mo with Appendix A (not the checklist) . Better request for it na para nakaready na pagdating ng PPR mo =) hehehe

hi sis...just sort of advise lang its better na u will use the lastname of your husband on your application para walang conflict process mo pero un nga lang u need to change all the necessary documents like passport.
Ung NSO ko took one and half month bago sila mag issue and saka pa lang ako nakakuha ng passport which is 2 weeks bago ka maissuehan ng new passport kasi required nila marriage license from NSO. If kukuha ka marriage license make it 3copies na agad...kasi u will need it to get your passport.

just wanted some clarification (though this may be a silly one) --- regarding the CENOMAR and AOM --- if i have submitted the CENOMAR in my application documents, does CEM still require to submit the AOM during PPR? coz i rechecked in the documents required, it states CENOMAR OR AOM.  and i think i have read somewhere in this forum that CENOMAR is for those who have no previous marriage, while AOM is for those who have.  Did i get this right?  sorry, just a bit confused. Sad thanks a lot in advance.

Cenomar is for singles while AOM is for married people.
Cenomar is issued to people who is never married.

So they would still request you a copy of your AOM with your PPR.

sis ung secpa, nkaprint cia sa security paper may pirma cia sa likod ng taga nso, ung certified true copy photocopy lang yata un.. pero sis try mo na pumunta sa nso, pag ngapply ka sabihin mo dun sa magveverify i ta-try mo lang kung meron na, susulatan nia ung paper mo ng TRY hehe:)
nagpunta ako dun 8am, nakuha ko ung marriage cert 11am, sana nga dat pa ko pumunta ng nso eh feeling ko mas makukuha ko ng maaga sa jan24 un..

nagpamedical na ko sis sa st.lukes extension clinic sa may bocobo, and i used my married name na din, nagparenew lang ako ng passport nakuha ko cia in 8 days lang mabilis nga eh kasi ung bnayaran kong rush 10days supposed to be eh ayun dumatng ng maaga..

immediately after the wedding kasi i started to work on the requirements, natapos ko ung mga reqs in a month.. so lahat nasend ko na kay hubby Smiley

try mo na sa nso sis pakita mo ung cert true copy mo Smiley
sana meron na din ung sa inio Smiley
