Hi all... Smiley

I am dependent of my spouse and i have some queries for form 5406.

when i fill this form for myself

1) Do i need to write my name as "Applicant" in Section A. or my spouse name who is  "Applicant" ?
2) how i can write NAMES in native language in that form do you have any tools to write it on ?
3) in SECTION D : (Signature)  I have to sign in or my spouse will sign it for me ?

pls need your gr8 help...

Hi,

You have to use separate forms IMM5406 for the Principal applicant and for the dependents.
As your spouse is the principal applicant write his/her name against Applicant and details in next columns
In the spouse column your name as spouse and details
In applicant's form give details of his/her family
In the  form  for spouse your family details as asked.

After printing, you can write the names in your native language legibly with pen.
If you think the space provided is not enough,  use a blank photocopy or blank print of same form and write the names only in native language and attached to the main form.

Hi wilson,

so according to you,
in my form (as depenedent) and my wife's form (applicant) , applicant name will be same.
and i will have to sign as well, right ? or my wife will sign both forms ?.

Principal Applicant > Your wife

 spouse>  Your name

In both forms section B ( children )  details will be same, if applicable

Section C will be different for wife and for you.

She will sign in her form  showing her parents her brothers, sisters

You will sign your form - showing your parents her brothers, sisters

Hi,
You should use 2 copies of IMM5406 for each applicant. (Dont confuse with terms Principal Applicant and Applicant).
In first copy, write all details in English, and in second copy write only the name in Native language using pen.

Each family member included in IMM0008 (Generic) /IMM 008 E (2D form) should fill in their own sets ( In English and Native Language) of IMM5406 as Applicant.

Your spouse (Principal Applicant) fill in the first set as "Applicant" and adds you as spouse; and adds all his/her Family Details( father, mother, brother,sister,kids etc.).

You fill in the second set as "Applicant" and adds your spouse in the "Spouse" column; and adds all your Family Details( father, mother, brother,sister,kids etc.).

Hi all

I think both of you Wilson and can1122 are helping me awesomely.

But there is a little bit confusion in "Applicant" name in both the forms from both of you (as per wilson "applicant" will be my wife in both form and as per can1122 "Applicant" will differ in both forms). so its little bit confusion.

one more question
1) how i need to send my documents to CIO ? . it should be attached with each form depending upon checklist or 1st will be set of all forms than all other documents ? ...

pls help me ... Smiley

Hi,
A family member included in IMM0008 (Generic) /IMM 008 E (2D form) is considered an " Applicant " for Immigration.
The main applicant in the FSW is termed "Principal Applicant" and rest all are termed "Applicants".

Every family member (" Applicant") has to submit his/her copy of IMM5669 E / Background information form.
In this form, every family member furnishes details of their background like schooling, personal history, addresses etc.
(In 5669, you may have noticed that the very first field is to choose whether the form is for Principal Applicant / Family Member of principal Applicant.)

Now, this family member has to provide information about his/her kids/brothers/sisters/parents etc.
To do this, the IMM5406, Additional Family Information is used.

So every family member has to submit their own copy of IMM5669 and IMM5406 as "Applicant".
This IMM 5406 form is an annexure to IMM5669 E Background information form, which is furnished for every family member.

The filed "Names" in IMM 5406 needs to be filled in Native Language as well.

You need to send all forms/documents to CIO; keep the filled in Checklist (IMM5612E) as cover page.

I have got perfect answer form you...

I know that i have to send all forms/documents to CIO  but my question is

do i need to attach all documents like "birth certificate , work experience letter etc "  separately after filling forms along with Checklist (IMM5612E)   OR i have to attach it along with forms where this information is filled (e.g. IMM 00008 SCHEDULE 3 - question 5. as IELTS result so do i need to attach IELTS result copy with this form ? )

pls can you send me your contact details .. if you can  Grin

Wishing you a Very Happy New Year!!!!

My suggestion for application package is:

1. All filled in forms and IMM5612 as cover page...
2. Other documents you can categorize for each applicant...like all your education docs bundled together, work experience together...
Make them in order so that its easy to find out....

Hope this helps...
Pls do a search in the forum and I am sure that you will find many more ideas...
