I'm helping my mother and she was wondering if she needs to take it before sending her application. And we are not going to use a representative so should we include the "Use of Representative Form"?. Should we leave it blank or put 'not applicable'. Thank you!

Ielts is very compulsory and there are minimum bands required depending on Occupation. If you are not using a rep then you don't need to include the rep form in your application.
