My parents receive their medical notice as well as requiring the following updated documents:

1. Schedule 1 form for you and all dependents over the age of 18, whether accompanying or not
2. Additional Family information form (IMM5406) for you and all accompanying dependents over the age of 18

I cannot find Schedule 1 from the link CIC has provided.

Is Schedule 1 form same as IMM 0008?

Not 100% sure so don't take this as fact, and anyone please feel free to correct me, but I believe Schedule 1 is now Schedule A - Background/Declaration (IMM 5669).
