I'm not sure where you came across the form called IMM 5662(At least i'm not aware of such a form) and also if you are asking about the Pre June 26 or Post June 26. If it is Post June 26, A full document and application set has to be sent to CIO, NS and that package should include the below forms.

1. IMM 0008
2. IMM 0008 - Schedule 1
3. IMM0008 - Schedule 3
4. IMM 5406
5. IMM 5620
6. IMM 5612
7. IMM 5476(Optional, use only if you have a representative)
8. VO specific checklist

Hi, indira,thanks for the reply on my post. yup, have to act quickly on the application. Although as of the moment, i am just a few documents still needed to complete the requirements. Just a few questions here, hope you and other members could answer. Sorry if the questions are redundant, its just that i havent found a clear answer.

1. In submitting the documents, did you make a seperate copy of documents needed for the CIO and another set of documents for the VO?

2. where do i put the original IELTS result since the requirements for the CIO and for the VO both need to have the original IELTS result attached to them?

3. What is the final word on the PCC? In the application to the CIO, it is said that we need to attach it while on the VO requirements, it is said that we dont need it yet? Should the PCC be original or a notarized copy would do?

Which VO is yours?

As for me, i arranged the papers in 2 lots, each with a checklist. The CIO checklist will go first and it should have ALL of the documents required in it, including the IELTS TRF. Next, take the VO checklist, fill it up(make sure you check ALL applicable columns even if you are providing the document in the CIO lot) and then arrange the rest of the papers. In short, i had 2 bundles, the first one CIO's and the second one VO's. The CIO will have ALL required documents and the VO will have the other supporting documents.

Just 1 copy of any document is sufficient as CIO will forward the entire set to the VO.

As for the PCC, where in the CIO checklist did you read it is required? If you see it, then it is the older version. Please download ALL the forms and guides as of today and then start working on it. PCCs are not required by CIO, if our application passes thro CIO, we will have to submit it to the VO. Please ensure your checklists will not have a place to indicate PCCs as an application just returned for a minor mistake of using the older version of the checklist where they had a space to check about PCC status. Good that you noticed and asked about this.

They require original PCC, but not now Don't wait for your PCCs to come! Send out the package ASAP.

Hi Raquel

Welcome to the thread. Consider CIO checklist as your primary set...and VO as secondary. So make CIO set with all the originals (as asked in their checklist) + photocopy of PA's PP (bio data pages only). Make the other set (VO specific) with the remaining / balance documents (as required in VO checklist) ..... for the documents that you have already attached in CIO set ....... just mention (with a note) that 'Submitted with CIO checklist'.
Don't forget to put the 'Tick mark' in the appropriate boxes in VO (& CIO too) checklist. All the Best.

If I'm payment through VISA I just need to fill and sign the PAYMENT FORM, right?
I dont have any way of doing the payment by VISA before I send the application... and they will pick the money from  my account when the application will be processed?

Welcome to the forum (& this thread!!!). I too had the same problem ... so what I did was this...

(1) Get a birth certificate affidavit through a notary. Make sure all the details are strictly as per your passport.
(2) Go to the following thread and take a printout of my post (reply #21). That post is actually a response from New Delhi VO that school certificate can be submitted / produced as a proof of date of birth. Just highlight the required portion. Along with this letter, attach a notarized photocopy of your 10th class pass certificate.

Dear all,

I need help . As i am under Backlog application ( Sept. 2006 NOC 3131 ).

Now i am planning to apply for Fast Track after going through number application filed till 26 June 2010 . Also 3131 is under new 29 Catagory.

But i have some questions on documentation requirements .

(1) Getting educational trascripts under university sealed envelop ? What exactly is the meaning of it ? what is the way to get it ? My educational is from three different universities . ( University of Rajasthan , Jaipur , IGNOU & ICFAI University New Delhi ) Any one have idea of any university how to get educational trascripts under university sealed envelop from these universities ?

(2) What are the exact documents required from my Employer ?

(3) 3131 is fall under FSW1 or FSW2 ?

(4) Do i need to send Proof of Fund with application ? If yes then what is way & how to present it to CIO ?

(5) Is it possible to that my OLD application & New Application run parallely ? When should i withdraw my OLD application ?

(6) Need to know how much points i will get from Education.

10+2 = Full time after that

2 year Diploma in Pharmacy = Fulltime

3 year Bachelor in Computer Application = distance mode from IGNOU , New Delhi

2 year Master in Business Administration = Distance mode from ICFAI University .Dehradun. 

My both the Distance university is UGC Approved .

Kindly Help to take firm decision to apply for Fast Track,
