Hi Qorax, Can i TAKE THE PHOTOCOPY OF THE iMMUNIZATION CARD WITH THE DOCTORS STAMP IN iT because then i don't have to do the writing work. or else can you please send me the pdf file or update it in the forum...thanks Thanks... yeah I saw that... I'll upload it's image instead in this thread, soon

As per CIC even a new born have to go through.IME.. but don't worry for infants and kids its just routine check... no test to be done |

But Qorax, lets say if doctor from the USA not signed third party or another forms which is not related to the own doctor groups, what to do? any idea? becasue in the USA doctors are not easily understand you or your situation, we have all immunization reports for my 2year 5months baby.

Once again Mr. Om Kumar, I don't seem to get your query... third party? another forms? own doctor groups? US doctors not easily understand u?1. If it's about who to sign the form/s ?-- It could be any RMP, a Paediatrician is better.2. If it's about whether to use this form ?-- No. It could be your kid's 'existing' Vaccination Card itself - with the sign, date & stamp of the medic.Hope I understood u, else kindly deliberate...my bad !Regards, Qorax |

Ok, but here in the USA if you ask to fill-out some forms which is not provided by USA doctors from your selected medical group or not from USA authority, than the hospital and doctor never sign any online printed forms. If I print above mentioned link form and ask to doctor for signature becasue I want to use it in Canada for my immigration and child shool admission related issue, I sure the doctor will not sign those form/s.
so any idea that if doctor not signed this form in the USA for my child so, what to do in this situation.
