Immigrant Summary & supplementary information form

Hello All

I am silent reader of this forum. This is my first post.
I have asked to submit Immigrant Summary & supplementary information form by
London CHC. I have few queries.

1- Do me and my spouse has to fill it twice?
2- I have 2yrs old child. Do we need to fill his detiails in Dependent column of this form Or just me and
my spouse details are needed coz "it is written that each family member over the age of 18yrs has to complete that form"?

you have to fill it just once

section (a)  is for you

(b) is for your spouse

2 year old child should not be included in this form, dependents of 18 years or older are required
