IMM5406 Additional Family Member and some other general questions

Hi all,
first of all, thanks for reading this post. It is very appreciated for all the reply
Q1: I am sponsoring my common-law to Canada, does my partner put my name down on the IMM5406 form as spouse/common-law? Or it only means previous spouse/common-law of her?
Q2: On the document check list, it mention that we need at least two statutory declaration from individuals. Is this just some sort of reference from your friends and families? (Or only friends, families dont count) Or it must be coming from notaries?
Q3: My common-law partner and I are living together in Canada at the moment, but we decided to process the application outside of Canada (people said that its faster to process outside of Canada, and CIC agent said its our option to do in-land or outside). In this case, does my common-law partner put down her residential address as her address in her mother country or our address in Canada?

ok for question number one she MUST put ur name down as her common law
question number two , its just some reference letters from ur friends or family its better from ur family
question number three , i would recommend if you are applying from outside canda then she needs put her mums address where she would be staying and if you are appling from withing canda you put ur current address , that s what i think .

Thanks a lot for your answer med:)
By the way, under IMM0008 Schedule 1 Background/Declaration section 9. What does it mean by 'Order to leave Canada' ? My partner had applied extending her visitor visa before, but it was refused by CIC and on their returning letter, it says that she must leave the country (because the time she applied extention is like 2 months before her visa expired, and it took CIC 3 months to process her case, thus by the time we receive the letter, her visa was already expired) Then, she left Canada and come back again after couple months. Does that considered as 'Order to leave Canada' ?
I read about Curina's cases saying that her husband get refused before, and end up has a warrant on him ??

ur partners case is not like cuina s case so dont worry no warrant should ever be on ur partner , u have mentioned that after this order to leave canada ur partner came back again to canada so where is the problem then , if there was any they would have stopped him or her in the aeroport , so dont bother by that sentence it doesnt apply to ur case , goodluck
