hi again,

I went to the doctor and he said to me that he doesn't have de IMM107EFC form.

But the problem is, how i ask for it, and where????

Was the doctor you went to on the list of acceptable immigration doctors? Just a thought.

yes, the doctor is an acceptable immigration doctor .
