for number "11. personal history"; i have been a self employed since 2003, should i leave the end date blank or put the date today?

what i mean is...i'm still doing my job unil now...what end date should i put into the "to" column" since i cant type "present" in it..should i leave it blank or put the date today?

you have two options,
Provide todays date or application filling date as the "Date" and then write a letter attaching to that form mentioning that for the most recent entry on question no A you have put this and that you are still employed /self employed.
option 2: put 'TO DATE' there and then attach a sheet to this form with that explanation ( i did this )

This topic has many cofussions.
However, i wrote the date of application "YYYY/MM" in "To________" part. I think it is more logical to write application date than writing To Date or To present.
Choice is yours.

Thats not how a lot of people I know have done it. Writing 'To Date' or 'Current' with a pen works just perfectly well.
