IMM0008 for spouse

NEED HELP

I submitted my application under skilled category. MARCH 2008

JULY 2008 got a letter from CHC that i have provisionally been  accepted and my medicals will be sent in a few weeks ALSO  they want me to send IMM0008 generic for my wife - while i am the principal applicant.

now i a confused: under which category must she apply - none written ther applies to her.

also in the family members that will accompany her,should she mention me and kids although they were including my sposue was mentioned in original application.

any experience on How long medical forms take to come.

Are you sure the requested IMM0008 Generic for your wife? As per my knowledge all applicants should have their spouse and other dependent family members over 18 to fill 'Schedule 1 Background Declaration' not IMM0008 Generic. If you have forgotten the 'Schedule 1 Background Declaration' for dependents, then it would be logic they ask you to send it.

no they specifically asked for IMM0008 generic for my wife too,  after telling me that my applications  provisionally accepted
