I don't think that's a standard form in a spousal sponsorship package and I don't think it's included in the document checklist so, no, if you have no children, you do not have to include it.

thanks for the reply.

another question from imm 0008 schedule 1 it says "Indicate whether you are 1. the principal 2. the spouse, common-law or dependent child aged 18 year or older of the principal applicant

my question is: i am sponsoring my spouse we dont have any dependent child or any other member included in our application, does he check the box 1 and 2? because im confused he is the principal applicant and he is my spouse as well.
need help for clarification in regards with this.

thanks!!

He is the principal applicant ONLY.  The part about "spouse, common law or dependent child of the principal applicant" refers to dependents, which he does not have.

HI

I don't think that's a standard form in a spousal sponsorship package and I don't think it's included in the document checklist so, no, if you have no children, you do not have to include it.

It is in the application pack for Skilled worker, so they either applying as a skilled worker or looking at the wrong set of instructions.
