imm 3911 SUPLEMENTARY INFORMATION FORM

COULD SOMEONE PLEASE TELL ME IF I NEED TO FILL IN SUPLEMENTARY INFORMATION FORM  PAGE 1 TO 4??


I am sponsoring my husband and we filled out all the questions except suplementary information form,education and employment history and military service. Because we did answered questions about this topics in other forms. I am originally from Turkey but live in Canada  and I am not sure if it applies to us.


I feel like I shouldnt fill it out because it asks for file number and we dont even have one. does anyone know that if this is a form to be sent by the visa officers after they see our papers?

IMM 3911 is the Document Checklist for your region.

You need to complete A-1 through A-5 to indicate that you're including the documents that apply to you, including the Checklist itself. And also the Supplementary Information form, pages 1 through 4 as stated in the Checklist, page A-1.
