I have a Question!! pls help

Today I checked my husband's application status and it stated
We received your application for permanent residence on May 6, 2009.
We started processing your application on May 26, 2009.
We received the requested information on September 21, 2009. Thank you for providing the information.
but we did not send any information, they did not even ask us for any information. We sent the medical exam along with the application.
May be the medical exams>?
Any ideas what it would be?

That seems odd, because for medicals they usually use "Medical results have been received."

May 12th - Passport with visa received
