How to reapply again for Canada

I have Applyed once in 1998 1999 From Germany and I had a possitive answer:So like "Aprowed Aplication Form"-and i had a bletter becomen that i had to go in Canadian Ambassy in Berlin.
I have that paper (aproved Aplikation form)still at home.   
Because my paper work was not ready I had to go back in my country -so i I didn't realise my Dream.................
