In education history, it is not possible to type university name in the box provided,,,can i write by ink pen,?Huh

In boxes  of date(from - to),,,if it not used,,can i write by pen n/a,,it is not possible to type.

Need to write some name n date by pen,,,,mixing of type and write,,,is it create any prob?Huh?

Thanks....

If the info is not getting fit in boxes of the forms, you can transfer your info on to another extra/additional sheet; give it #, write your family/given/full name, form #, Qeust #, and attach it with the form. Writing with printer+pen can creat prob.

fill as much as you can electronically. then take a print out and write the fields left with black ink pen.

There r many (From -to) box,,,which is not required for me,,,can i write n/a with pen?Huh??

Type current month/year (11/2009) but do not leave it blank.
This will be applicable to "To" column only.

do you have to fill deceased box, even he/she is not deceased? I meant, do you need to put N/A there or just leave it blank?
