As part of my child's immigrant visa, I have got a form to be completed so that my baby can attend school at Canada. This form will state all Vaccination details. Can some one guide me on how to get this completed at UK.
Will the red-book for the baby, that has all Vaccination detail replace the above form?

Any one who has got the child immunisation form signed in England can guide me on how to do that?

Well from what I have read on this forum, the form still needs to be completed and signed by your GP or a nurse (not sure what they call them anymore in UK. Left about 10 years ago). But the form just confirms immunizations received as in your red book. Hope this helps.

I have my boy's red books and have got copies of our medical records from our GP, I'm hoping this will be enough proof !
