Hi guys good morning,

Just want to ask regarding changing of my spouse name after marriage.
My wife change her driver's license due to we moved to other province and she'd like to change her last name too
(Last name after marriage)and the officer made her last name like this.

Name before marriage/Maiden Name:

Juana Doe Smith

Juana Doe Smith was married to Jonh Delacruz, so the name should be, Juana Smith Delacruz right?(Correct me if im wrong)

But the officer insist it should be like this.

Name after marriage:
Juana Doe Delacruz

It makes us wonder. Any suggestions pls?

If her family name is Smith in her maiden name, she should change it to your family name which is Delacruz ( if it is your family name )
Therefore now your wife's name should read: Juana Doe Delacruz.

i agree with ugly venus because

first name : Juana
middle name: Doe (from the mother)
Last name : Smith (from the fathers last name)

when married

first name: Juana
maiden name: Smith
married name: delacruz

Are your married in phil or here in canada? It best to change name when renewing passport. And the rest will follow

Hi that's my case too. I'm from BC before and moved here in Ontario. My Drivers Licence in BC is still under my single name (because I'm not married yet when I first got my Licence) and then when I moved to Ontario I want to used my married name but they want me to carry my mother's maiden name. I got my new WORK PERMIT under my married name and my Newly Issued Passport under my married name too but unfortunately Service Ontario told me  in Canada that's not how they change it unlike in the Philippines they'll change your middle name as your father's last name. That's why I just decided to hypenated my name. Otherwise they said I need to seek a Lawyer to legally change my name.

Sample to make it clearer:

SINGLE NAME

First Name: Honey Lynne
Middle Name: Dela Cruz
Last Name: Pascual


MARRIED NAME/NEWLY ISSUED PHILIPPINE PASSPORT NAME/NEWLY ISSUED WORK PERMIT/OHIP CARD
FIRST NAME: Honey Lynne
Middle Name: Pascual
LAst Name: Reyes

ONTARIO DRIVERS LICENCE NAME:
PASCUAL-REYES, HONEY LYNNE DELA CRUZ

Note: I used sample name for privacy purposes. If above name exists pardon me.

Just to make sure that you know to make sure you have your marriage certificate! Some places will demand (not ask, demand) one.

The name change after marriage is optional, not required.

I am not sure what province you are in, but here is the link to Service Alberta.  If you are not in Alberta, your province registry would be the place to go to process the name change.  Either way, there is a lot of good information on the Service Alberta site.
