how do i send medicals and passport together

Dear All
My VO is New Delh.Just would like to know .Medical is sent by DMP,then how do i send passport and medical together(if doctor is gonna send medical). And the address to send documents is Immigration Section - 7/8 Shantipath, Chanakyapuri PO Box 5209 New Delhi 110021.

Also im suffreing from Soar throat and cough is it going to be problem for medicals?

plz chek the address  and advise me .All replies /views appreciated

The address to send passport is correct, delete the Post Pox. I will suggest to send the passport after 10 days of medicals, sometimes the doc calls again for any test and the passport is required for that. Put a sticker on the passports - mentioning your name and file number.
 infection can effect your medical test, do tell it about it to your DMP along with the drugs name.
