We put everything in 9x12 envelopes and labeled them by category. Some people do file folders and rubber bands. Bottom line is we had a box 5 inches deep and 11.5 pounds.

Try to be as neat and organized as possible.

Hi  ,

No specified forms. You can use your own method in a systematic way. Take standard A4 size white (normal typing size) paper and paste over it. Make a set of all different attachments in a file nd clip together with your details on the file. You can itemize and mark over that about the contents.Ex:
contents:  Phone bills ...
               ticket
                photographs
               .......

There is no hard and fast rule for such things. Should be in a presentable manner.
