when they ask for Given Name and Family Name....
this is what I did...but am curious which line do i put the middle name on (first or second line of given name):
names changed below to protect the innocent (LOL)

family name:  Smith
given name(s):  Thomas John (both on same name - first and middle names)

or should it be...

family name: Smith
given name(s): Thomas
John (this being on the second line on that box)

Putting it on the same line with a space between both first and middle name is how it is usually done.

look at the passport...see how it is written... if the middle name is under given name on the passport ..then write it that way...
arrgghhh... I was under the same confusion...shld we put the middle name...should we not...finally we decided to go with how it was written on the passport( they also mention in the forms to write the name as it appears on the passport)

It's funny how a simple thing like this could get us so confused...I'm so glad I'm done with filing the application Smiley

i agree with what brin said.. the name has to be exactly as it appears in the passport .. this is sometimes overlooked

and agreed filling the forms is a huge burden that's done!!!

I think the second line is for people with very long names or with several middle names that won't all fit on the first line.

Oh me too! Smiley My husband has a middle name, an initial and a traditional Joseph after he was baptised. There is no occasion when he applies for a piece of paper, document, card whatever, his name appears correct. Our son has only one name for good reason.Smiley
