help with medical forms please!!

morning all
i am applying outland through london uk for family sponsorship, having recently being married in ontario.
i have 3 children, aged 21,17 and 13 who are also being included in the application.
the medicals are booked for next week, they are asking for me to bring with me (amongst other things)

section A or appendix D

i phoned the designated medical practitioners and asked her where/what these are?
once informing her that i was applying family class she said i needed to bring
Appendix C instead


ok i may be being thick (heh) but these are the forms i have so far...

app for perm residence in canada IMM0008 (04-2010) E
sponsored spouse/partner questionnaire IMM 5490 (04-2008) E
AND 3 additional family family information forms IMM 5406 (04-2008) E
ONe for each of the children

what is this appendix C ?
where do i obtain it?
any other forms needed?
very confused here

It should be in the region specific forms.
Don't always believe what the doctors or their staff tell you, BTW. They frequently mix up the Federal Skilled Worker requirements and the spousal requirements.

Part 1 has all the sponsors forms, Part 2 the Applicants forms and Part 3 is region specific.  You will find in there is the medical form Appendix C
