Help with form imm 5406

Please I will like to know if i can add the name of one of my cousins in the Form Imm 5406? he his my fathers younger sister's son, we do not bear the same surname but he attended the same schools and lived together.He has lived with my parents before he turned 2yrs,  and he his a few years older than I am, I see him more has a brother than a cousin but i hope a cover letter will surffice letter to explain the relationship?
